VOLUNTEERING AT LEIDY'S CHURCH

Leidy’s Church is thankful for the support of its many volunteers. Our volunteers are the reason that we’re able to provide great children’s programs! All individuals volunteering with children under the age of 18, in any capacity at Leidy’s Church are required, by the state of Pennsylvania, to complete the following clearances and forms:

1. PA CRIMINAL BACKGROUND CHECK

2. PA CHILD ABUSE HISTORY CLEARANCE

Click here to complete the PA Child Abuse History Clearance.

From this site, click on the "Create Individual Account" button. The site will guide you on how to create an account (username, password, etc.).

3. AFFIDAVIT or FBI CRIMINAL HISTORY BACKGROUND CHECK

Complete ONE of the following requirements:

If you have lived in Pennsylvania for the last 10 consecutive years please complete and sign the Leidy's Act 153 Volunteer Affidavit.

If you have lived outside of PA at any time in the last 10 years, click the following link to schedule an appointment for fingerprinting to complete the FBI Criminal History Background Check. Use 1KG6ZJ as the Volunteer Service Code. Please be prepared to bring a driver’s license or passport to your appointment. Payment of $23.85 is payable at the time of service. Upon request this fee can be reimbursed with a receipt submitted to the church office.

Once you have completed the necessary documents, you should submit all paperwork to the Leidy’s Church Office in person or email to office@leidyschurch.org. Please retain original copies for your records.

*NIGHT TO SHINE volunteers should send their clearances to bucksmontnts@gmail.com

If you are volunteering for more than one position at Leidy’s Church these clearances will cover all volunteer positions. Submit your clearances to the church office and your information will be on record. Please note, all three clearance reports must be renewed every five years.
